Update to current CXML.
* types.lisp (SAX:START-PREFIX-MAPPING, SAX:END-PREFIX-MAPPING):
Changed from primary methods to :after methods.
* validate.lisp (VALIDATOR): Inherit from default-handler rather
than sax-parser-mixin. (TEXT-NORMALIZER): Don't inherit
explicitly from sax-parser-mixin.
