Make @fnindex available for all formats.
Currently these @fnindex entries were only available for info files
(and the text help): because they were wrapped in @ifinfo/@end ifinfo.
That would mean they couldn't be found in the html files.
To make the help consistent, make these available for html (and every
other format) too.
